What Is NOT covered?

  • ZEISS HD no line and ZEISS HD single vision ordered prior to March 1, 2016
  • Progressive or Single Vision lenses outside the HD family of lenses
  • HD2 Safety lenses
  • Optician errors and Doctor remakes

What to do after submitting the form?

  • A ZEISS customer support specialist will contact you within 1-2 buisness days with confirmation of return authorization.
  • The return authorization form with shipping instructions will be faxed to your store. Send the frame and defective lenses with the RA form to the address provided.
  • The complete pair with new lens will be shipped back to your store in 7-10 business day.
